Volunteer Cheque Form Available Online
Parents can now download and print off a copy of the Volunteer Form from the Parents tab on the Website. There will be copies available at time of tryout. Please remember to bring the $30 tryout fee and your Cheque for $500 made payable to MTMHA.
Be sure to include all participants from your household on the form and submit with the eldest.
