By agreeing below, I am confirming that all of the above information is accurate and true.

This application is being made in good faith with the understanding that a false application may result in any future consideration or application for coaching (or other MTMHA staff) positions being rejected.

If selected for an interview I may be required to furnish references to support my application.  If awarded a coaching position I will make every effort to fulfill the obligations of being a coach and adhere to the policies of MTMHA.

I understand that an OPP Vulnerabilty Sector Check must be successfully completed by interview date or have a valid VSS (thus signing a declaration at the time of interview) to be considered.  I will also complete the OMHA Respect in Sport Leadership Program as necessary.
